Both the 'Effusion Time' and 'Determining Molar Mass' examples require solving an equation with the unknown trapped under a square root — squaring both sides of 1.66/1 = √44.0/√M to solve for M is exactly the radical-equation technique, and skipping the squaring step (or squaring only one side) is the standard place this goes wrong.
